Horizon Sports Physio

These Terms & Conditions apply to physiotherapy and rehabilitation services provided by Horizon Sports Physio. By arranging and attending an appointment, you agree to the terms outlined below.
Appointments
Appointments with Horizon Sports Physio are arranged directly following an enquiry. Appointment length will depend on the type of appointment booked. Please arrive at the agreed time to allow the full appointment to be provided. If you arrive late, it may not be possible to extend your appointment beyond the original finishing time.
Fees and Payment
Current appointment fees are displayed on the Horizon Sports Physio website and will be confirmed when your appointment is arranged. Payment may be made by cash, bank transfer or another payment method agreed at the time of booking. Payment is normally due on the day of the appointment unless otherwise agreed. Horizon Sports Physio reserves the right to amend appointment fees. Any changes will not affect appointments that have already been agreed at a different price.
Cancellations and Missed Appointments
We understand that plans sometimes change and will always try to accommodate appointment changes where possible. Please provide at least 24 hours’ notice if you need to cancel or rearrange your appointment. Appointments cancelled with less than 24 hours’ notice may be charged at the full appointment fee. Missed appointments without notice may also be charged at the full appointment fee. Exceptions may be considered at the discretion of Horizon Sports Physio where there are exceptional circumstances.
Physiotherapy Assessment and Treatment
Physiotherapy assessment and treatment will be tailored to your individual presentation, goals and circumstances. Treatment may include advice, education, exercise rehabilitation, manual therapy and other appropriate physiotherapy interventions. Your treatment options will be discussed with you and you are encouraged to ask questions at any stage. You may decline or withdraw consent for any aspect of assessment or treatment at any time. Physiotherapy outcomes vary between individuals and no specific treatment outcome or recovery timeframe can be guaranteed.
Extended Specialist Assessments
A longer appointment may be recommended where additional assessment time is considered appropriate, for example for more complex presentations, multiple areas of concern or where a more detailed specialist assessment is required. Where possible, this will be discussed with you before the appointment is arranged.
Online Consultations
Online physiotherapy consultations are available by prior arrangement where clinically appropriate. You are responsible for ensuring that you have a suitable internet connection and a safe and private environment in which to participate. There may be circumstances where an online consultation is not appropriate and an in-person assessment or onward referral is recommended instead.
Children and Young People
Appointments for children and young people will be managed in accordance with appropriate professional standards relating to consent and safeguarding. Where appropriate, a parent or legal guardian will be required to provide consent and/or be present during the appointment.
Clinical Records and Confidentiality
Appropriate clinical records will be maintained in accordance with professional and legal requirements. Personal and health information will be handled confidentially and in accordance with applicable UK data protection legislation. Communication
Email and other electronic communication may be used for appointment arrangements, rehabilitation information and other matters relating to your care. While reasonable measures are taken to protect confidentiality, standard electronic communication cannot be guaranteed to be completely secure. Please avoid sending sensitive information through methods you do not consider sufficiently private.
Referrals and Other Healthcare Professionals
Where appropriate, Horizon Sports Physio may recommend that you seek further assessment or treatment from your GP, consultant or another healthcare professional. Information will only be shared with other healthcare professionals where there is an appropriate basis to do so, as explained further in our Privacy Policy.
Emergencies
Horizon Sports Physio does not provide an emergency medical service. If you believe you are experiencing a medical emergency, please contact the appropriate emergency medical service rather than waiting for a response from Horizon Sports Physio.
Website Information
Information provided on the Horizon Sports Physio website is intended to provide general information about the services available. Website content does not constitute an individual medical diagnosis or replace assessment by an appropriately qualified healthcare professional.
Responsibility During Rehabilitation
You are encouraged to inform your physiotherapist of any significant changes in your symptoms, health or circumstances that may affect your treatment. Exercises and rehabilitation advice should be followed as instructed. If an exercise causes unexpected or concerning symptoms, you should stop and seek appropriate advice.
Limitation of Liability
Horizon Sports Physio will provide services with reasonable care and skill in accordance with applicable professional standards. Nothing in these Terms & Conditions excludes or limits liability where it would be unlawful to do so.
Complaints and Concerns
If you have any concerns about your care or experience with Horizon Sports Physio, please contact us in the first instance so that the matter can be discussed and, where possible, resolved.
Changes to These Terms
These Terms & Conditions may be updated periodically to reflect changes to our services, policies or applicable requirements. The latest version will be available on the Horizon Sports Physio website.
